I recognize that the Springfield Men's Soccer League (SMSL or the "League") plays in places and uses facilities which may be open to people and/or not associated with the SMSL. I understand the need to behave in a responsible and adult manner when engaging in SMSL play, associating with formal or informal SMSL activities, and when utilizing public or private facilities and I will make my best efforts to exhibit appropriate behavior, including regulating the use of vulgar or offensive language. I agree to abide by the rules, regulations, and decisions of the SMSL governing game play and player actions. In consideration for being permitted to participate (including but not limited to playing, watching and traveling to and from events) in the Springfield Men's Soccer League (SMSL) I hereby agree to accept any and all risks of injury or damage. I agree that neither the SMSL, SMSL board, the Springfield Area Soccer Association (SASA), the Springfield Park District, nor any of their affiliated entities or individuals, directors, officers, employees, agents, volunteers or any other representatives shall incur any financial responsibility or liability whatsoever for any injury or damage resulting from negligence or nondeliberate act, however caused. Accordingly, I hereby release each of the foregoing individuals and entities from all actions, claims or demands that my heirs and assigns or I may have for injury or damage suffered or incurred by me due to my participation in the SMSL.  If this form is being completed on behalf of anyone who will be 17 years of age as of the start of the season I certify that I am the parent or guardian of the player registering and that I consent to my child's participation in SMSL activities. I have read and I understand the above contract terms. In consideration of allowing my child to participate, I consent to the contract terms and agree that its terms shall bind me, my child, my heirs, legal representatives, and assignees.
